Dear Parents, Carers and Specialist Schools,

 

Re. SEND Health Hub – Self-Referral from Families

 

Since May 2020, services across Children’s Community Health and CAMHS, have worked together to provide a service to offer advice and support to families during the ongoing pandemic. We’ve called this service the SEND Health Hub. Referrals received by the team are considered on a weekly basis and following a discussion, a member of the team, which could be a therapist or CAMHS practitioner, makes contact with the family to discuss their situation and support needs.

 

Whilst referrals up to now have been from special school staff and health workers, we are now pleased to be able to receive referrals directly from families. There are several ways to make a referral for a child or young person you care for:

 

  • You can email a straightforward self-referral form to the team at mft.manchestersendhealthhub@nhs.net
  • You can contact the SEND Health Hub on 0161 230 0301
  • Or, you can speak to your local Special Needs School Nurse who can complete a referral on your behalf.

We have enclosed copies of the service information leaflet and the self-referral form with this letter. If you would like any further information about the SEND Health Hub or would like to discuss a child you care for, you can contact us via the telephone number or email address given above – and a team member will contact you.
